 Mono / Stereo recordings?

Most recordings are technically supposed to be mono, aren't they? But oftentimes the recordings are in 2 channel stereo. They shouldn't be, unless someone is using a dual microphone setup, but who does that?

Are you talking about voice recording? Yeah, those should be in mono, unless you're recording multiple people (ie. a choir, a capella group). I'm not really sure what you're asking, but I guess sometimes people fiddle with settings when they don't need to and end up with a dual mono stereo recording, which is essentially pointless.

However, stereo recording is AWESOME for some applications. I love recording acoustic guitars in stereo (at least 2-3 mics) to get a nice stereo image. For example, I've been really hooked on the mid-side recording technique. One cardioid mic, and one figure-8. Double the figure-8 track, flip the phase on one, and you end up with a recording where you can actually fiddle with the stereo spread. So much fun.

For voice tracks, they should always be mono because, well, a stereo voice track, is just a mono voice track that takes up more space. Like, it doesn't add anything. If a director wants to then turn it into stereo, for effects and what not, they then do that.
